Call a private prosthetist from our list to make an appointment. Tell them you have a Metro South Oral Health Voucher.
At your appointment you will need to show:
Without these documents your treatment cannot go ahead.
After treatment your prosthetists will ask you to sign a copy of the voucher. This shows us that the work is finished, and that you are happy with the treatment.
Before you sign the voucher, talk to your prosthetist if you have any concerns about the treatment you received.
You can call the practice you visited if you have trouble with the fit of your denture or, if you are concerned about its quality.
If you cannot make it to your appointment, call the practice to let them know.
